The Lucknow-Mumbai Pushpak Express was halted after an alarm chain was pulled, prompting the train driver to activate the flasher light and apply brakes as per protocol, officials said. However, a curvature of nearly two degrees obstructed visibility and shortened the braking distance, contributing to the tragic accident.

The driver of the Karnataka Express, noticing the flasher light, also applied brakes but could not prevent the collision. Railway officials noted that trains in this section, part of the trunk route, typically operate at speeds exceeding 100 km/h. A full-speed train requires at least 650 meters to come to a complete stop after braking. Despite both drivers following protocols and making every effort to avoid the accident, the incident resulted in the loss of 13 lives and left 10 others injured.

The accident occurred between Maheji and Pardhade stations near Pachora town in Jalgaon district, North Maharashtra, around 4:45 PM. Railway officials confirmed the tragedy happened after the Pushpak Express stopped abruptly due to the chain pulling.

On January 23, 2025, ex-gratia payments were distributed to nine injured passengers of the 12533 Pushpak Express at Vrindavan Hospital and Vighnaharta Pachora. Five individuals—Hasan Ali, Vijay Kumar, Uttam Harjan, Dharma Sawant, and Abu Mohammad—received ₹50,000 each for grievous injuries. Four others—Moharam, Hakeem Ansari, Deepak Thapa, and Hujala Sawant—received ₹5,000 each for minor injuries. The total compensation distributed amounted to ₹2,70,000.
